Our dedicated team includes experts in Mental Health General Practitioner services, offering personalised and compassionate care for individuals facing mental health and physical health challenges. Additionally, our center boasts specialised expertise in Child and Youth Psychiatry, ensuring that young patients receive the specialised attention and support they need for optimal well-being.
Moreover, we are excited to introduce an Integrated Care Pathway, designed to streamline and enhance the overall patient experience, promoting seamless collaboration among our medical professionals.
At Omega Wellness Centre, our commitment is to provide high-quality, integrated healthcare to empower individuals on their journey to well-rounded health and recovery.
Patients with both psychiatric disorders and chronic physical health conditions, can now be referred to the Omega Wellness Centre’s Integrated Care Pathway (ICP).
This collaborative community care model between Omega Wellness Centre and Omega Mind Clinics, involves the management of patients by Senior Consultant Neuropsychiatrist, Dr Jasvinder Singh and General Practitioner Mental Health Specialist, Dr Sonia Res.
Referrals from Consultant Specialists and General Practitioners will be accepted on a case-by-case basis, and patients will be seen over several months by the ICP team at Omega Wellness Centre.
Patients will receive an initial comprehensive assessment with Dr Res for diagnostic clarification, investigations as clinically indicated and commencement of a treatment plan. Following the initial appointment, patients will be seen 4-6 weeks later by Dr Jasvinder Singh for further diagnostic and management consolidation.
Appointments will then alternate between Dr Res and Dr Singh, providing a unique care model of clinical expertise. The OWC Allied Health team may also be involved in the care and management of patients with access to nurse practitioners, psychologists and dietitian in 1:1 or group settings.
An Multidisciplinary Team Meeting (MDT) will be scheduled during the fourth month of care, to discuss the patient's current treatment plan. This meeting will involve the patient, carer, referring General Practitioner and other health team members, where appropriate.
We would expect most patients to be discharged from the IPP Clinical Pathway within 12 months of referral. Correspondence letters will be written to the Referring doctor.
The IPP Clinical Pathway will be evaluated after one year to see how it can continue to improve to try to meet these community needs.
Your initial appointment with Dr Sonia Res will be $350, with a Medicare rebate of $118.00. This appointment will be 40 to 45 minutes, followed by discussion of management plan, arranging any necessary investigations or referrals to our allied health team.
Your initial appointment with Dr Jasvinder Singh will be $500, with a Medicare rebate of $247.25 (60 minutes).
Subsequent appointments will vary between Dr Sonia and Dr Jasvinder Singh (Neuropsychiatrist) as required for your treatment plan. Appointments with Dr Sonia Res will be either $110, with a Medicare rebate of $41.40 or $180, with a Medicare rebate of $80.10 (dependent on the length of the appointment). Follow-up appointments with Dr Jasvinder Singh will be either $315, with a Medicare rebate of $126.40 or $225, with a Medicare rebate of $82.15 (dependent on the length of the appointment).
